Any group, organization or individual who is planning to operate a food booth during a fair, festival, auction or other temporary event must obtain a Temporary Health Permit.
All applications for Temporary Food Service Permits must be received at least five business days prior to the scheduled event. The fee for all Temporary Permits is $50. You may make application by printing and completing the application below. You must also contact Business Licensing at 325-7079 to obtain a letter verifying your business license or exemption status. The completed application and verification should be mailed or delivered to the Independence Health Department at 515 S. Liberty Street, with the $50 fee. You can also fax the application and verification to (816) 325-7074, and we will collect the $50 fee over the phone with a credit or debit card.
Routine inspections of food establishments is an ongoing process. All food establishments in the City of Independence are routinely inspected one to three times per year depending on Public Health risk. However, if the Health Department deems it necessary, establishments may be inspected with more frequency.
Critical Violation: A violation that relates directly to factors which lead to foodborne illnesses.
Non-Critical Violation: A violation that relates to maintenance and cleanliness of food service operations.
All critical violations are corrected within 72 hours by food service establishments.

Food Safety Training Classes are offered in English, Spanish, Chinese (Mandarin), and Sign Language for Hearing Impaired. English classes require pre-registration and pre-payment of the $15 fee. You may pre-register with a valid credit or debit card by using the online system or by calling (816) 325-7803. If paying with cash or check, please visit us at the Independence Health Department at 515 S. Liberty Street during normal business hours.
Participants receive a Food Handler Permit after successfully passing a written test. These permits are valid for three years and may be renewed online.
Duplicate or Transfer Permit fee is $5. You may obtain a permit Monday through Friday at the Independence Health Department, 515 S. Liberty Street, Independence, MO.
Food Handler classes last approximately one and a half hours.
Food Manager Classes last approximately four hours.
Food Handler classes are $15 and Food Manager classes are $30. Volunteers will be asked to verify where they are doing their volunteer work, and their cards will be printed as "Volunteer". "Volunteer" cards are not accepted in paid positions as a "Food Handler" card. Volunteer registrations are no longer accepted online. To register as a volunteer, you must call (816) 325-7803, or visit the Independence Health Department at 515 S. Liberty Street in Independence, MO.
Registration fees will not be refunded. If you cannot attend the class for which you pre-paid and you reschedule by at least noon the day prior to the class, the pre-payment can be applied to one rescheduled class.

Food Manager classes are held at the Independence Health Deparment at 515 S. Liberty Street, Independence, MO. A SERVSAFE certificate is acceptable in place of attending this class for the purpose of obtaining a Food Manager's Card.
